How do I change my name once I've submitted my application?
If it is necessary to request a name change, please submit the completed form below via email to grad.ask@grad.gatech.edu. Attach a copy of one of following:
• Court-ordered name change document
• Birth certificate
• Marriage license
• Divorce decree
